Reporting Hate & Discrimination

Victim Services provides support to victims of hate. The below form allows you to report acts of hate as well as choose to identify yourself if you wish to receive support.

The support provided includes:

  • Providing emotional support

  • Providing community resource information related to social services, religious institutions, and community groups.

  • Provide support when a police report is filed (you do not need to file a police report).

 

To report a hate-motivated act or to request support, please complete the form below.

Acts of hate may include the following that target a particular person or group based on race, national or ethnic origin, religion, age, sex, sexual orientation, gender identity or expression, marital status, family status, genetic characteristics, or ability.

  • Verbal abuse/harassment (including offensive jokes, name calling, targeted insults)

  • Intimidation or bullying (in-person)

  • Physical assault

  • Vandalism, destruction, damage or harm to personal property or belongings

  • Graffiti

  • Online abuse

  • Hoax calls, abusive phone calls or text messages

  • Malicious complaints

  • Displaying or circulating discriminatory posters or literature

 

An act of hate and discrimination does not need to target an individual. 

 

An example may include discriminatory messages or symbols painted on buildings or memorials.

Trends regarding the types of incidents, groups being targeted, and where the incidents are taking place will be shared with community partners, identifying information will be removed and shared as a statistical summary.

Specific identifying information about the individual(s) or incident(s) will not be shared with police or anyone else without your consent, unless there is a direct risk of harm to self or others.  (Duty to Report and Protect)

 

This form is not considered a police report. If you wish to file a police report, please contact your local police agency or attend police headquarters. Please note that as this form is not a police report, it cannot directly lead to formal charges. Depending on the information provided if the victim wishes to provide contact information, they may be encouraged to make a police report if a crime has occurred.

Information collected is submitted to our office, and every effort will be made to respond within three business days (Monday – Friday, 8:30am – 4:00pm, excluding holidays). If you prefer to leave a voicemail, please call our office at 705-360-8700.

This form is anonymous by default, but if you would like to provide your information to receive tailored supports, please opt in at the end of the form.

If you are a service provider or filling out the form on behalf of someone else, please fill in the demographic data for the person(s) that were targeted.
